Required
- Hold a full driving license with access to a car – due to working across dual locations
- Must be registered with The Nursing & Midwifery Council, upholding the Codes of Conduct
- Must have a Registered PIN Number

Duties Include
- Overseeing complex care packages
- Undertaking holistic person-centred assessments and reviews
- Developing and implementing plans and ensuring information is shared with the team to promote positive outcomes for the customers we support
- Working collaboratively with other professionals to ensure individuals’ care needs are met and maintained
- Undertaking staff competency assessments working closely with the nurse consultant
- Contributing to monthly on-call, one weekend per month and one evening per week.
